Location: Wisconsin Rapids, WIFamily Health Center of Marshfield, Inc. ("Owner") intends to develop, design, construct, equip, and operate a new outpatient healthcare facility in the City of Wisconsin Rapids, Wisconsin (the "Project"). The Project is anticipated to be a singlestory facility of approximately 24,000 gross square feet. Owner plans to provide dental, primary care, pharmacy, behavioral health, and substance abuse services in the facility. All of Owner's services within the Project will be provided on an outpatient basis and, therefore, the building is anticipated to be an occupancy classification of "Business - Group B". Owner is in the process of acquiring a land parcel in the City of Wisconsin Rapids ("Property") to develop the Project.
Luther Group, LLC ("Luther Group") has been retained by the Owner as their owner's representative to manage the land acquisition, development, design, and construction process of the Project.
A construction manager has not yet been selected for the Project. Owner and Luther Group are currently undergoing a separate proposal process to select the construction services team. Construction services and any trade work are not included in this request for proposal ("RFP").
The project will include funding from the USDA Rural Development Community Facilities Program and the American Rescue Plan Act of 2021. All drawings, specifications, schedules, and material legends to be produced in a manner that ensures full compliance with Build America, Buy America (BABA) requirements for iron, steel, manufactured products, and construction materials. All specifications require bidders to submit product origin documentation acceptable to USDA. If any required materials are identified as non-domestic or uncertain in origin, the Design Team shall assist the Owner and Construction Manager in preparing the market research, cost comparison, product availability, and origin documentation necessary for a USDA RD BABA waiver submission. Provide reasonable support to the Owner and Construction Manager in developing documentation needed to satisfy funding and reporting requirements associated with governmental funding used for this Project.
As this project will utilize USDA Rural Development Community Facilities funding, proposers must comply with the following:
a. This procurement will be conducted in a manner providing full and open competition consistent with 2 CFR 200.319. No restrictive requirements (e.g., proprietary specs, unnecessary experience or bonding, geographic preferences unless permitted) will be used.
b. Firms or individuals that were involved in drafting this RFP, the scope of work, or project specifications are prohibited from competing, as required by 2 CFR 200.319(b). All proposers must disclose any real or apparent conflicts of interest per 2 CFR 200.318(c).
c. Proposers must not be suspended or debarred and must have an active registration in SAM.gov at time of award, consistent with 2 CFR 200.214.
d. The Owner will maintain detailed records of the procurement, evaluation scoring, negotiations, and award documentation as required under 2 CFR 200.318(i). Proposers shall provide supporting documentation upon request.
e. Consistent with 2 CFR 200.321, proposers shall support the Owner's efforts to include small, minority-owned, women-owned, and labor-surplus-area firms in procurement processes and subconsultant opportunities.
f. All specifications prepared by the Design Team must comply with 2 CFR 200.319(d), including clear technical requirements, minimum essential characteristics, and "brand-name-or-equal" descriptions where appropriate.
Family Health Center of Marshfield, Inc. reserves the right to reject nonconforming, non-responsive, or conditional bids.
